To continue and finalise your registration you must read and agree to the following terms and conditions:
PAYMENT TERMS AND CONDITIONS
The minimum notice of cancellation required is four (4) working days prior to this program. THIS IS NON-NEGOTIABLE.
Payments will only be refunded if students provide official medical documentation.
PARTICIPATION RELEASE AND WAIVER OF RESPONSIBILITY
I acknowledge and understand that:
o By attending this program, I am doing so voluntarily and at my own risk.
o I understanding that I am representing Deakin University and as such will behave inline with the student code of conduct.
o I take full responsibility for all of my actions, including the actions that may result in injury, loss or damage.
o DUSA has a ZERO TOLERANCE POLICY regarding the use of illegal drugs and does not knowingly allow the provision of alcohol to minors.
o NO ALCOHOL IS PERMITTED ON THIS PROGRAM
o I give my consent to receive treatment from a legally qualified practitioner should an emergency situation arise.
o I am responsible for any cost associated with any necessary medical treatment.
o Photographs taken by an official DUSA photographer may be used in DUSA promotional material.
o Any misconduct or refusal by me to follow legitimate instructions provided by an official DUSA staff member may result in the cancellation of my participation in the activities, and even removal from the camp.
COVID SAFE - TERMS AND CONDITIONS
By attending this DUSA event, you agree to adhere to the Victorian Government, DUSA and venues COVID-related restrictions and requirements that are in place at the time of the entry, inclusive of the below.
· You are declaring you are well and without COVID symptoms. If you’re unwell, you should not attend the event.
· You will record your attendance via the check in QR code or locally advised system to support contact tracing, and adhere to space capacity constraints.
· After attending the event you test positive to COVID-19 you will notify DUSA via 1300 555 528.
· You will follow all COVIDSafe instructions as sign-posted and advised by designated individuals of the Venue and DUSA.
· The host of the event (DUSA) and the venue, reserve the right to postpone or cancel the event in the circumstance that COVID-related restrictions and requirements render the event not feasible. DUSA will act in the best interests of all ticket holders through consultation with our venues to ensure the safety of patrons, in the unlikely circumstance that the event is cancelled a refund will be provided to the ticket purchaser.
· Every intention of the organiser (DUSA) and our partners, contractors and suppliers is to host this event in the safest way possible for all attendees. If we need to cancel or postpone the event we will communicate this to you as soon as practical.
Should the practical component of this program be cancelled/altered due to COVID restrictions, the online event will go ahead. |
